Bile that has thickened behaves like cold oil

Fat-triggered bloating gets treated as a bacterial problem. Sometimes the issue is further upstream, and it is physical.


The standard sequence for someone with bloating after rich meals is probiotics, then enzymes, then an elimination diet, then a cupboard of half-finished bottles. All of those interventions target the lower gut. None of them touch the fluid arriving from above.

Bile has to be thin to work

Bile is a surfactant. Its mechanical job is to take large fat droplets and break them into microscopic ones so that pancreatic enzymes can reach them. To do that it has to arrive, in volume, at roughly the moment food enters the duodenum.

Bile can thicken. When cholesterol saturation rises and mucus, crystals and cellular debris accumulate, the fluid becomes viscous and sediment collects. A gallbladder full of thickened bile empties poorly when it contracts, and less surfactant reaches the food.

Fat that is not emulsified is not efficiently digested. It travels further down the tract, where it contributes to loose, greasy, poorly formed stools and to gas production as gut bacteria act on material that should have been absorbed higher up.

The measurement

A controlled trial in patients with symptomatic cholesterol gallstones measured the physical properties of gallbladder bile after treatment with hydrophilic bile acid therapy at 750 mg daily.

Treatment produced a significant decrease in gallbladder bile viscosity and reduced the total sedimentable fractions, the thick sludge made up of precipitated cholesterol crystals, mucus and debris. Cholesterol crystal observation time was prolonged.

Two things about that study are worth being straight about. It was conducted in gallstone patients, not in people with unexplained bloating and clear scans. And it measured the physical properties of bile, not symptoms. It establishes that hydrophilic bile acid therapy thins viscous bile. It does not establish that thinning bile resolves anyone's bloating.

Why the distinction is worth keeping

The viscosity mechanism is a reasonable explanation for why upstream problems produce downstream symptoms, and it is supported by direct measurement. Whether it is the explanation for your particular symptoms is a separate question, and one that a clear ultrasound does not settle in either direction.

What the evidence supports is narrower than the marketing around it: bile can thicken, thickened bile flows badly, and hydrophilic bile acids measurably thin it.
